To update the DOB on your Upstox account, you’ll need to follow the steps below. This process requires submitting physical copies of specific forms and supporting documents via courier.

Note:

• Please submit each request on a single modification form, and avoid combining multiple request types on the same form. This will help us process your requests efficiently.
• Please fill out a separate form for each request. Don’t include different kinds of requests on the same modification form. For example do not submit Name and Signature modification using one form, use 2 different forms.
• Each request will be processed one after the other, with an individual turnaround time of 2–3 working days.
• When submitting a DOB Modification request, do not include any address information in the modification form.
• Only mention the specific details that need to be updated and leave all other fields blank, as instructed.
• Changes related to address, email, or mobile number must be submitted online through your Upstox profile.

Step 1: Update Your DOB in the Income Tax Database (ITD).

If your DOB is already updated on your PAN Card, proceed with the steps below.

Step 2: Submit the Required Form and Documents.

1. Download and fill out the Modification Form: Click here to download the form.

2. Attach a Self-Attested Copy of Your PAN Card:

• Ensure the PAN card reflects your updated Date of Birth. (This is mandatory and please sign within the white space provided on the PAN card)

3. Details to Fill in the Modification form:

• Upstox Client ID: Mention your unique ID.
• Basic Details: Enter your name as it appears on your PAN Card.
• Photograph: Attach a passport-sized photograph with your cross-signature.

Step 3: Courier the Completed Form and Documents.

Send the completed Modification Form along with the required proofs to:
