Steps to Remove or Update a Nominee:

Step 1: Download and Print the Form

• Download the Change of Nominee Form.
• Print and fill out the form carefully.

Step 2: Courier the Completed Form to the following address:

Information to Fill in the Form

Ensure that you fill in the following details correctly:

1. Name

• Enter your name as per Income Tax (IT) records.
• Click here to verify your details.

2. Upstox UCC

• Provide your Upstox Unique Client Code (UCC).
• Learn how to check your UCC in the Upstox app.

3. To Remove a Nominee

• Mention the existing nominee's details on Page 4 (Account Details Modification Form).

4. To add a New Nominee

• Provide the new nominee's details on Page 4 (Account Details Modification Form).

• If the nominee is a minor, complete the nomination and guardian details on Page 1 (Nomination Form).
