Under Rule 158 (12) of the Income-tax Rules 2026, the Director General of Income-tax (Systems) is required to specify the forms and formats along with the procedure for submitting PAN correction forms.

In an official order dated April 1, Manu Malik, Director General of Income-tax (Systems), said the Permanent Account Number (PAN) holders are required to fill the following forms for changes or corrections in PAN data:

  • PAN CR-0 I

  • PAN CR-02

Individual PAN holders can request changes or corrections in PAN data by applying through the PAN CR-01 form.

Non-individual PAN holders can request changes or corrections in PAN data by applying through the PAN CR-02 form.

The CBDT has also shared PDF formats of both forms, along with related guidelines (See here).

The PAN correction form for individuals seeks details such as name, gender, date of birth, full address, passport number, taxpayer identification number in the country of residence, contact details including mobile number, landline(if any), email ID, father's name, and mother's name.

The new PAN correction form also asks taxpayers to specify whether they want to print their father's or mother's name on the PAN card. It is mandatory to select one of the parents, whose name is to be printed on the PAN card.

The applicants also need to submit documents such as proof of identity, proof of address, proof of date of birth, proof of change in support of the proposed changes/corrections.

If applying offline for correction, the tax department suggests filling the form legibly in block letters and preferably in black ink. Further, the form should be filled in English only.

Applicants are required to mention their PAN correctly. In case of loss of PAN, a copy of the FIR should be submitted along with this form. It is also mandatory to submit Aadhaar, except for the exempt category.

How to submit new PAN correction forms

With effect from April 1, 2026, PAN holders can submit these forms physically in the PAN Centres of M/s UTIITSL/M/s Protean eGov or online through their websites, the CBDT said.
